Declarations by working students
This page offers information to those who have been a working student at HvA or a related program. On the right side of the page, you will find a form called an IB47-form. Fill it in as carefully as possible.
Be sure to include:
- Your complete IBAN and BSN numbers
- When explaining your assignments, include a description of the activity and the dates involved
- With the time period, include begin- and end-dates
- It’s not necessary to fill in the lines for field of study and the name and number of the cost centre
- Do not forget to fill in your name and leave your signature on the bottom left.
Submitting the form
Once you have completed the form, you can hand it in at the Educational Office. Don’t forget to include a copy of your proof of identity with a visible social security number (i.e. you may have to copy both sides of the document!).
Incomplete form
With forms that are incomplete or incorrectly filled-in, you will need to provide the missing information. Until that time, payment is not possible.
Payment
You will receive the owed amount in your bank account in approximately four weeks.
